The requirements of the Affordable Dwelling Unit Program shall apply to any site... which is (a) served by public water and sewer, and (b) the subject of an application for rezoning, special exception, site plan or preliminary subdivision plat which yields... twenty-four (24) or more dwelling units at an equivalent density greater than one unit per gross acre.
